Anti-Müllerian Hormone (AMH) is the single most commonly ordered fertility blood test — and the one that causes the most anxiety. A low AMH result can feel like a sentence. But AMH measures ovarian reserve (how many eggs remain), not egg quality. Understanding what your number means in context — including what it doesn't tell you — is essential before making treatment decisions.

What AMH Tells You
Quantity, not quality. AMH correlates with the number of antral follicles in your ovaries — the pool of eggs that could potentially respond to stimulation. It tells your reproductive endocrinologist how many eggs they're likely to retrieve during an IVF cycle, which determines protocol design (high-dose vs. low-dose stimulation, antagonist vs. agonist protocol).
IVF response prediction. An AMH of 3.0 ng/mL at age 34 suggests a strong response to stimulation — likely 15–25 eggs retrieved. An AMH of 0.5 ng/mL at the same age suggests 3–6 eggs. Both patients can achieve pregnancy; the lower-AMH patient may need more cycles or a modified protocol to accumulate enough embryos.
What AMH Does NOT Tell You
It doesn't predict natural conception. A low AMH doesn't mean you can't conceive naturally — it means you'd produce fewer eggs under IVF stimulation. Women with very low AMH conceive spontaneously every day. AMH is an IVF planning tool, not a fertility crystal ball.
It doesn't measure egg quality. A 30-year-old with an AMH of 0.8 ng/mL has fewer eggs than average but her egg quality is still that of a 30-year-old — which is excellent. A 42-year-old with an AMH of 2.0 ng/mL has a good number of eggs, but age-related quality decline means a higher percentage will be chromosomally abnormal. Quality is age-driven. Quantity is AMH-driven.
It fluctuates. AMH can vary by 20–30% between tests, between labs, and even across your menstrual cycle (though less than FSH). A single low reading is not definitive — repeat testing at a reliable lab before making major decisions.
